One of my coworkers, a CAD instructor, just got a 3D printer (Dimensions Elite), and of course, being a nerd, I went to town. I so owe her materials for what I plan on putting this machine through.

Anyway, I printed three different versions:
1/16th inch inset for acrylic plate
1/8th inch inset for acrylic plate
2-part model, that would use screws, plus what I will use to cast the blue lens

We may need to modify the models a tad, to increase space between the curve at the bottom of the tag. If you look where I circled in red, you'll see what I mean; there's a gap. The wall edges are just close enough to have the printer not add material there.

The bonus of this is, they're super durable. I chucked the 1/8th inch inset model across the room, and it barely took any damage. Dropping from shoulder height onto solid floor did nothing. ABS plastics all the way!
